• Copy of the statement that shows holdings of the shares you'll be transferring. If the shares are held physically, a copy of the face of the certificate(s) will suffice
• Two pieces of ID from you (driver’s license or passport + 1 other, such as utility bill/mobile phone bill/bank statement/etc)
• Copy of the form that will need to be completed as part of the transfer. Complete the form, but do not sign it
